    INTRODUCTION

    As the advancements of technology make themselves applicable in various fields, psychiatry too has not been left untouched. It has become even more promising as the application has spread to the use of technology in serving health care to remote and inaccessible areas. This technological upheaval is important to psychiatry as it can be a necessary vehicle to bridge the prevalent mental health treatment gap. Disasters and pandemics pose unique challenges to providing health care. During COVID-19 pandemic, travel restrictions, inaccessibility to healthcare facilities and reduced in-person interactions has forced clinicians and service users to depend on Telepsychiatry services heavily. Though Telepsychiatry might not solve them all, it is well suited for scenarios in which mental health professionals can evaluate and manage patients. Centre for Tele Mental Health was started in the month of May 2020 due to prevailing COVID 19 pandemic.

    AIMS

    The centre aims to provide synchronous as well as asynchronous Telepsychiatry services in the following forms:

    • Patient consultation and follow-up services.
    • Therapeutic services including prescription of medication, psychological and social support
    • Research and evaluation
    • Remote monitoring of patients needing supervision at home, drug side-effects etc.
    • Establishment of consultation – liaison services
    • Setting up of mobile vans/telepsychiatry clinics at remote locations (under-served and at- risk population) to increase the penetration of psychiatric care within the community
    • Linking up the existing community extension clinics and school mental health program with telepsychiatry services (Rural telepsychiatry & Child and Adolescent telepsychiatry)
    • Establishing public-private partnerships with NGOs, trust hospitals, local organisations working in mental health but lacking the expertise in the management of patients with psychiatric illnesses
    • Imparting education to postgraduate trainees in various specialties of mental health (Tele- Education)
    • Imparting emergency telepsychiatry services in the event of natural or man-made disasters

    STEPS TO BOOK AN ONLINE TELECONSULTATION

    Step 1

    Visit the Site https://ors.gov.in/orsportal/

    Step 2

    Click on Book Tele-consultation Appointment

    Step 3

    Select the State Jharkhand

    Step 4

    Click on Central Institute of Psychiatry CIP

    Step 5

    Then two options will be displayed on the Screen as “Tele-consultation for Follow up patient and View Appointment Status”

    Click on Tele-consultation for Follow up Patient

    Step 6

    Click on Proceed button after reading the instructions carefully

    Step 7

    Enter UHID NO and the caption given below in the box

    Step 8

    You will receive an OTP on your registered mobile number

    Step 9

    Enter the OTP and follow subsequent steps as mentioned in instructions

    Step 10

    Your Appointment is booked for Tele-consultation and a confirmation SMS is also sent to the registered mobile number. This message implies that an appointment request has been sent to CIP tele-consultation team for further confirmation.

    Step 11

    When the hospital confirms the appointment booked by the patient from ORS, the patient will again receive an SMS on the registered mobile number with the date and time of tele-consultation along with video-conferencing link and the video-conferencing pin which will be required to join the teleconsultation session

    Step 12

    On the day and time of the appointment confirmed by the hospital, the patient can join the teleconsultation session using the link and pin mentioned in the confirmation SMS received.

    This service is available to those registered clients who have undergone at least 2 in-person Psycho Therapy sessions at CIP.

    Tele-MANAS

    The National Tele Mental Health Programme of India, Tele Mental Health Assistance and Networking Across States (Tele MANAS) envisions to work as a comprehensive, integrated and inclusive 24 x 7 Tele-mental health facility in each State and UT in India with the specific aim and objectives. The apex institutions, regional co-ordinating centres and mentoring institutions closely work with the states/UTs in all these activities by providing support for technical issues, collaborative consultations, capacity building, implementation, service provision, linkages, monitoring and evaluation, research, innovations etc.

    CIP, Ranchi is one of the 23 Mentoring Institutions and one of the 5 Regional coordinating centers (RCCs) in the country. State Tele-MANAS cell of Jharkhand is also operational at Centre for Tele Mental Health, CIP, Ranchi and is currently operating 24x7.

    Aim: To provide universal access to equitable, accessible, affordable and quality mental health care through 24 x 7 tele-mental health counselling services as a digital component of the National Mental Health Programme (NMHP) across all Indian States and UTs with assured linkages.

    Objectives

    • To exponentially scale up the reach of mental health services to anybody who reaches out, across India, any time, by setting up a 24x7 tele-mental health facility in each of the States and UTs of the country.
    • To implement a fully-fledged mental health service network that, in addition to counselling, provides integrated medical and psychosocial interventions including video\ consultations with mental health specialists, e-prescriptions, follow-up services and linkages to in-person services.
    • To extend services to vulnerable groups of the population and difficult to reach populations.

    During COVID 19 pandemic, Jharkhand CSO Forum set up a Telemedicine facility to reach out to the needy persons in remote areas especially those who are in home isolation and can't afford and access to such services in the restricted mobility situation. CSO Forum signed a non-financial MoU with CIP in 2021 to address mental health issues for the persons attending Telemedicine facility run by CSO Forum. These services were provided by Centre for Tele Mental Health, CIP. Apart from addressing mental health issues CIP was also involved in IEC activities for activists of CSO forum.

    CIP has signed a non-financial MoU with WHP for their projects Comprehensively Address Mental Health Issues Related to COVID-19 Pandemic (CAMH) and Closing the Gaps in TB Care Cascade (CGC) in the month of July 2021. The major objectives of the project are early screening, diagnosis and management of mental health issues of COVID-19 and TB patients and their family members, including gender-based violence along with engagement of community to adopt preventive behaviour, addressed through tele-counselling, tele-medicine, linkages and follow up visits. Central Institute of Psychiatry through its Centre for Tele Mental Health is providing guidance and technical support in the form of capacity building, training and skilling of human resource in mentoring project team, private providers and other stakeholders to improve project outcome. CIP is also providing treatment and management to cases referred by CAMH and CGC project as per the patient need and requirement.

    e-Sanjeevani - National Telemedicine Service of India is a step towards digital health equity to achieve Universal Health Coverage (UHC). e-Sanjeevani facilitates quick and easy access to doctors and medical specialists from smartphones. People can also access quality health services remotely via eSanjeevani by visiting the nearest Ayushman Bharat Health & Wellness Centre. It enables virtual connection between the beneficiary at Wellness Centres and doctor and specialist at the hub. It was launched in November 2019 to provide quality healthcare to the people residing in remotest regions. These services are part of the larger government scheme to connect larger hospitals to smaller health centres in remote areas. As it expands, medical college hospitals and large government hospitals in the States would act as ‘hubs’ to provide tele-consultation services to ‘spokes’, or primary health care centres (a “hub and spoke” model).

    CIP, Ranchi is currently functioning as a HUB for the field of Psychiatry in Jharkhand under e-Sanjeevani initiative. Through this portal, Community Health Officers (acting as SPOKES) from all over Jharkhand assess the patient need for Psychiatric consultation and initiate telepsychiatric consultation with CIP. Teleconsultation through E-SANJEEVANI is operational everyday on all working days between 11am-1pm at Centre for Tele Mental Health, CIP, Ranchi.

    The joint collaboration of Ekjut and CIP,Ranchi from 27-07-2021 is reducing the large mental health care gap in rural parts of Jharkhand. The approach not only takes care of medical access but also helps in reducing stigma and discrimination in mental health issue.

    The initiative includes:

    • Collaborative Tele- psychiatry + treatment
    • Assistance to support group meetings for service users and caregivers

    MoU signed between CIP and EKJUT on 27/07/2021

    Tele-consultations have been provided to patients since March, 2020. To coordinate tele-mental health services effectively, the Centre for Tele-mental Health was set up in June, 2020 and has been operational since then. Tele-mental health services have been provided to a total of 593 patients during the period under report.
